A health assessment is the assessment of a patient’s health, physical, psychological, and social function. It involves the consideration of whether preventive health care and education should be offered to the patient.

The following categories of health assessments may be undertaken by a medical practitioner with the support of practice nurse in accordance with the Department of Health, the following health checks could be performed for:

  • 45-49 years who are at risk of developing a chronic disease

  • a type 2 diabetes risk evaluation for people aged 40-49 years with a high risk of developing type 2 diabetes as determined by the Australian Type 2 Diabetes Risk Assessment Tool

  • a comprehensive medical assessment for permanent residents of residential aged care facilities
