OUR DOCTORS ARE ALL HIGHLY QUALIFIED CONSULTANT – LEVEL RACGP SPECIALISTS
Doctors consulting at this hub are all highly qualified and experienced consultants – Level RACGP Specialist. Their consulting fees are MIXED BILLING.
The appointment you book has set time aside for you to discuss your concerns with your doctor. This means that your doctor will not be available to see another patient. If you do not attend your booked appointment, a ‘Did Not Attend’ Fee (DNA fee) applies.
Consulting Doctors have allocated Standard Appointment for 10 minutes. If a longer Appointment is needed, please inform the staff, or select longer appointment at HotDoc.
It is the patient’s responsibility to remember their appointment date and time.

CONSULTING DOCTOR’S BILLING CRITERIA
The criteria:
- Children under the age of 16 years
- Government initiative for Preventative Health Assessment with the following categories:
* People age 40 to 49 years for Diabetes Type II Risk assessment.
* People age 45 to 49 years who are at risk of developing chronic diseases.
* People aged 75+
* People who have intellectual disability.
* People who have served the Australian Defence Force.
* Indigenous Health Assessment
* People who are on a humanitarian visa with Medicare access including refugees and asylum seekers.

NEW PATIENT
• Any patient who has not attended the hub in 12 months is considered a new patient.
• All new patients must pay for their first consultation in accordance with Doctor’s Fees Policy, except for children under 16 years of age.
• Medical records from previous practice MUST BE TRANSFERED to this hub to enable continuity of care by the consulting Doctor.
WOUND CARE
• Fees apply depending on the time used for the provided service.
RESULTS, REPEAT PRESCRIPTIONS, REFERRALS
• A GAP Fee applies for each consult depending on the time used for provided services.
• If the Consulting Doctor refers you for further investigation (Pathology, Radiology) or to a Specialist or to an Allied Health Practitioner – check their billing policy when you are making your appointment.
CONCESSION CARD HOLDERS (Health Care Card and Pensioner Concession card holders)
• All Concession Card Holders must pay a discounted fee.
• Fees will be charged depending on the time consumed with the consulting Doctor.
• All Commonwealth Seniors Health Card holders will pay the usual ‘Out of Pocket’ Doctor’s fee with no Concession.
DID NOT ATTEND – DOCTOR’S FEE POLICY
• If you do not need your booked appointment, please cancel your appointment at least 4 hours before your booked appointment time.
• An invoice of $50.00 will be emailed to the patient who booked and Did Not Attend (DNA Fee) their booked appointment.
• No further appointment will be made until the invoice is paid in full.
TRANSFER OF MEDICAL RECORDS
• Patient’s medical record is kept safe, confidential, and secured.
• If you request the transfer of your medical record to another medical facility, an Administrative Fee of $50.00 will apply to facilitate the transfer process.
• Patient’s Health Summary will be transferred with no charge.
TELEHEALTH CONSULTATION
• Doctor’s consultation over the phone or video will be treated as the Consulting Doctor’s Fees, based on the duration of the attendance.
• Patient must have visited a doctor at this hub in the last 12 months to be eligible for a Telehealth appointment, otherwise, the patient will be asked to book face-to-face appointment or be billed privately with no Medicare rebate.
• Doctors’ consulting at this hub encourages a face-to-face consultation, unless the patient is not physically able to attend or is sick with Covid or has other reasons preventing from physical attendance.
MINOR PROCEDURES
A GAP fee of $40 applies for Skin Biopsy, Ingrown Toenail, and any other minor procedure that requires a doctor and a Nurse service.

WORKCOVER CONSULTATION
Patient with NO CLAIM NUMBER – Must pay Doctor’s consultation fee on the day.
If Workcover claim is rejected, the patient Must pay Doctor’s consultation fee.
SKIN CANCER MANAGEMENT
• Initial Doctor’s consultation to be booked for screening and planning.
• Separate consultation to be booked should a procedure be required.
• Mixed billing will be charged for each Doctor’s consultation.
AFTER HOUR TELEHEALTH SERVICE – BILLING CRITERIA DOES NOT APPLY
MEDICAL REPORTS
• Consulting Doctors can complete medical reports for insurance claims, legal claims, Superannuation, Commercial Driver license renewal, or for any other services not covered by Medicare.
• Private billing payment to be made on the day for such reports.
• Ask reception staff about the fees to be charged when you are making your appointment.
• Private billing applies for a supervised RAT Test for COVID-19 Travel Clearance Certificate.

Caloundra Health Hub is open Monday to Friday from 8am to 4:30pm.
Weekends and public holidays is available via Phone Consultation only (Mixed billing fees apply).

• You can choose to book a specific type of consultation that you need, and the time you require.
AFTER HOUR MEDICAL SERVICE
• After-hours is a Telehealth Mixed Billing Service.
• Call the practice number 07 5438 1488. Your call will be triaged and diverted, your details will be collected and passed onto a doctor who will contact you through a private number.
• Consulting Doctor After Hour Fee is $100.00 per consult.
• Consulting Doctor Public Holiday Fee is $120.000 per consult.
• Urgent After Hour Fee is $210.00 per consult.
