Please note that we are now a Mixed Billing Practice. BULK BILLING services are available for:

– Children under 16
– Holders of Health Care Card, Pension Card, Concession Card
– Aboriginal and Torres Strait Islander Patients
– Chronic Disease Management
– Mental Health Care Plan
– Health Assessments for Preventative Health Care

We have a standard out of pocket gap of $35 payable on the day.

Our practice provides patients with adequate information about our practice to facilitate access to care. Our doctors give patients sufficient information about the purpose, importance, benefits and risks associated with proposed investigations, referrals and/or treatments, to enable patients to make informed decisions.

Our practice has a wide range of health promotion provided by our clinical team aiming at preventative health care of possible diseases and has specialised services to provide quality improvement for one-year cycle of care to our patients suffering from asthma, diabetes, cancer, cardiovascular diseases.

Our practice also provides additional services: child immunisation, travel vaccination, seniors health, skin cancer checks and minor procedures, mental Health, women health, pregnancy and post-natal care, family planning, contraception planning including Implanon insertion & removal, men’s health, adolescent health, medical reports for Workcover, lawyers, pre-employment, private driving license.

Management of your personal health information

Your medical record is a confidential document. It is always the policy of this practice to maintain the safety and security of all gathered personal health information and to ensure that this information is only used to provide accurate medical diagnoses and appropriate treatment.

Getting the results of any investigation

Your doctor will advise you when they expect your results to arrive at the practice. Give us a Call on 5438 1488 to make an appointment to discuss your results regarding your healthcare.

Our practice has a standard ten-minute appointment. Longer consultation times are also available on request, so please ask our receptionists if you require extra time. If you or a family member require an interpreter service or if you have a special need, we can organise this for you. Please let us know when you make your appointment.

Emergencies will always be given priority and our reception staff will attempt to contact you if there is any unforeseen delay.

Cancellation Policy

Our practice has a $75 Cancellation fee for Failure to attend or cancellation without at least 4 hours notice. This is payable before any more appointments can be made.

Home and other visits
Home visits are available for regular patients of this practice whose condition prevents them from attending the surgery.
Telephone access

GP’s in the practice may be contacted during normal opening hours. If the GP is with a patient, a message will be taken and the reception staff will advise you when it is likely that the GP will return your call. In an emergency your call will always be put through to a GP.

Your rights

Our practice always provides respectful and culturally appropriate care to all patients under all circumstances. Please let us know if you have a specific need.

The presence of a third party observing or being involved in the clinical care during a consultation occurs only with the permission of the patient prior to the consultation.

Our practice provides opportunities for and responds to patient feedback. Patients can provide feedback or address grievances via a suggestion box which is situated in the waiting room. This practice has an information sheet that encourages patients to give their positive and negative feedback. Despite the best intention’s, complaints may arise. If you have a problem, we would like to hear about it. Please feel free to talk to your doctor or to the receptionist or you may contact us at: admin@caloundrahealthhub.com.au

We take your concerns, suggestions and complaints very seriously. The feedback obtained is then passed onto the Practice Manager.

Smoking Policy
Smoking is not permitted in any area of this Practice.
Reminder system

Our practice is committed to preventative healthcare and we use HotDoc online booking system, to assist with the communication with our patients and improve practice efficiency.

We may issue you with a reminder notice from time to time offering you preventative healthcare services appropriate to your care. This could be through a letter, an email, a telephone call or via an SMS message.

(If you do not wish to be part of this system, please inform your doctor or the reception staff about your decision.)

Request for Repeat Prescriptions

Our Practice Requires that all patients be seen by a doctor for repeat prescriptions. The only exception is if the Doctor agrees to write a script without a consultation.

Requests for Referrals

Our practice has a policy that requires all patients to be seen by a Doctor for obtaining referral letters.

Care outside normal opening hours

Caloundra Health Hub has a formal contract with “House Call Doctor” an after-hours home doctors’ service on Sunshine Coast. Their contact number is 13 55 66. This valuable service will then provide a consultation report to your doctor to ensure the continuity of care towards your health.
